Background: In forensic psychiatric care, coercive measures and prolonged hospitalization affect patients’ autonomy and motivation. Motivation is central to the recovery process but presents a challenge for both patients and nurses. Aim: To describe experiences motivation in forensic psychiatric care, from the perspectives of both patients and nurses. Method: A qualitative design with an inductive approach was applied. Data were collected through semi-structured interviews with six patients and six nurses. Results: The analysis of the patients' experiences resulted in four categories: The desire for progress drives motivation, Being influenced by the content and structure of care, Having social interaction as a driving force for motivation, and Being personally affected by motivation. The analysis of the nurses' experiences revealed four categories: Patients need to be on board with their journey, Being part of the mutual dynamic in motivation and care, Patients being limited by the forensic psychiatric structure, and Creating the journey together with the patients. The analysis process culminated in a shared theme: Meeting in change – a mutual journey toward motivation. Conclusion: Motivation is a complex and reciprocal process, influenced by structural, relational, and individual factors. A person-centered approach and trust in the care relationship are essential to promote motivation within forensic psychiatric care.
