    Abstract [en]

    Developing a program theory for a project and evaluate the project based on this program theory has both its merits and limitations. These merits and limitations in turn may be depend on the existing attitude towards a particular program theory. This study is about exploring how different phases and parts of the national learning project Care on equal terms (2011-2014) functioned in relation to the program theory developed in the beginning of the project. The result shows that the program theoretical perspective on the project contributed to a better structuring of the various forms of evaluation, given a broader understanding of the project's components and provided better opportunities to improve monitoring of the project by the research team in collaboration with the project management of the Swedish Association of Local Authorities and Regions (SALAR) and the participating health care units.

    Unaccompanied asylum-seeking children coming to Sweden are almost all fleeing from violent or threatening situations in their home country and have been sent abroad for their protection. They form a specific growing group of children looked after by local authorities in kinship foster families, network foster families or traditional foster families, as well as in small institutions (HVB-hem) or interim accommodation (Mellanbo) as preparation for independent living. This study explores success factors and impeding factors in a social work intervention for empowering unaccompanied children at a support centre for daily activities in a Swedish municipality from a program-theory perspective. The unaccompanied children in this study are staying in foster families. The data was collected at the centre and with a mixed method design. The results show that unaccompanied children’s relationships with other children improved; they developed their ability to plan and adapt; some of them started to attend regular schools; and traumatic experiences were processed to some extent. All of these can be interpreted as increased empowerment in unaccompanied children’s everyday life. However, there were still significant gaps in participation in clubs and associations and leisure activities, and some children were mentally unwell. Success factors, challenges and risks coexist in a transnational life situation in which most unaccompanied asylum-seeking children continue to interact with their parents and other family members in their home country or another country using modern technology (online parenting).

    Abstract [en]

    Unaccompanied asylum-seeking children coming to Sweden are almost all fleeing from violent or threatening situations in their home country and have been sent abroad for their protection. They form a specific growing group of children looked after by local authorities in kinship foster families, network foster families or traditional foster families, as well as in institutions (HVB-hem) or interim accommodation (Mellanbo) as preparation for independent living. This research study explores some daytime activities within social work for unaccompanied children at a support centre, Solitario. The centre existed for three years and some of the activities are now included in regular activities by the local authorities supporting unaccompanied children. The methods used in this study were 12 in-depth interviews with children, in-depth interviews with all staff at the centre and systematic longitudinal assessments performed by staff of 17 children attending the centre in 2009 as well as in 2011. The results show that there is a breakdown of the structures and communities that should support the unaccompanied child in the everyday life. There is not good enough cooperation and coproduction in supporting the best of the child in line with United Nations’ Convention on the Rights of the Child (UN CRC). The basic idea of UN CRC is that the child should be seen as an independent subject and actor. Because of their living situation, unaccompanied children are forced to take action and to do so independent of their microsystem – their nuclear family – or with just a trans-national support in web-based microsystem relationships.

    Hur fungerar det kunskapsbaserade brottsförebyggande arbetet?: En processutvärdering av arbetet med lägesrapporter och medborgarlöften i Fagersta och Ludvika kommun2021Report (Other academic)
    Abstract [sv]

    Syftet med denna processutvärdering har varit att kartlägga och analysera arbetet med lägesrapportering och medborgarlöften i Fagersta och Ludvika kommun. I linje med utvärderingssyftet ovan har frågeställningar som undersökts i processutvärderingen för respektive insats i båda kommunerna varit följande: Den första har handlat om vilka framgångsfaktorer, risker och hinder var som funnits utifrån de uppsatta målen och den andra om vilka förbättringsförslagen har varit för respektive insats.

    Det empiriska materialet består av intervjumaterial och dokumentation av insatserna i Fagersta och Ludvika. Utvärderargruppen närvarade vid möten i EST-gruppen och lokala Brå i Fagersta, samt FAS-gruppen och lokala Brå Ludvika. Vid dessa tillfällen presenterades utvärderingens upplägg och planeringen för intervjuer. Därefter planerades intervjuerna. Åtta fokusgruppsintervjuer (med sammanlagt 34 deltagare) och tre individuella intervjuer genomfördes. Samtliga intervjuer genomfördes via Teams på grund av rådande omständigheter med Covid-19. Intervjuerna utfördes utifrån en anpassad intervjuguide bestående av semistrukturerade frågor.

    Resultaten från processutvärderingen visar att likheterna mellan kommunerna överväger, men det finns också skillnader. I såväl Fagersta som Ludvika kommun utförs ett målinriktat brottsförebyggande arbete och det finns goda förutsättningar att stärka detta arbete. Utifrån resultat och analys av intervjuer kan ett antal slutsatser dras om framgångsfaktorer, risker och hinder, liksom möjliga förbättringsförslag identifierats avseende arbetet med lägesrapporter och medborgarlöften i berörda kommuner. De centrala slutsatserna för det pågående brottsförebyggande arbetet utifrån analysen av intervjumaterialet är följande:

    * Arbetet med lägesrapportering har stärkt kommunernas kunskap om sina respektive lokala utmaningar* Kommunernas brottsförebyggande arbete skulle stärkas ytterligare av en mer tydlig och sammanhållen process* Återkopplingen till lägesrapporteringsgrupp och rapportörer behöver utvecklas* Kommunernas kan ytterligare stärka sitt förebyggande arbete genom att tydligare kommunicera de åtgärder som genomförs

    Abstract [en]

    Change-projects can be evaluated in different ways. One of the ways is to evaluate with process thinking, which means that evaluators are involved in the process from the very beginning. Involvement of evaluators already in the beginning of a project can mean several challenges for both the project itself and the evaluators. This study is about describing and analyzing how and with what results the perspective of process thinking on evaluation of the national learning project Care on equal terms was used. The results indicate, among other things, that it was a good choice to bring in evaluation perspective at the beginning of the project. Process thinking has, among other things, contributed to the activities throughout the project, and played a role in learning from and implementation of the project itself.

    Background: In June 2011 the Swedish government signed an agreement with The Swedish Association of Local Authorities and Regions (SALAR), for a three year project to develop and implement “Health care on equal terms.” The project, which involved seven Primary Health Care Units (PHCU) from five county councils in different parts in Sweden, was completed in early 2014. The aim of the project was to develop methods and activities that could promote more equal health care provision in socio-economically disadvantaged areas.

    Aim: To assess and compare health care providers’ experiences of and perceptions about equitable health care at the beginning and end of the national project “Health care on equal terms”.

    Methods: A web survey was sent to all staff at the seven participating Primary Health Care Units (PHCU) at the beginning (2012) and the end (2013) of the project. Data were analyzed with descriptive statistics and the open issues with content analysis.

    Results: In 2013, the percentage of health care providers who reported thinking patients’ ethnicity had no or very little impact on access to care increased, but the proportion of those who reported that they had “no idea” that patients’ gender, age, mental health and physical functioning were significant for access to care was lower in 2013 than in 2012. The results from analysis of the open-ended questions did not show meaningful changes in the respondents’ perceptions of the issues addressed in 2012-2013, but the analysis contributes to a deeper explanation of the answers.

    Conclusion: The main conclusion is that it was possible to implement changes aiming for more equitable care through projects with a focus on learning.

    Introduction Danish and Norwegian immigrant women in Sweden have an increased risk of cervical cancer compared to Swedish-born women. In addition, Danish and Norwegian immigrant women follow the national recommendations for attendance at cervical screening to much lesser extent than Swedish-born women. The aim of this study was to explore how Danish and Norwegian immigrant women in Sweden reason about attending cervical screening, focusing on women's perceptions as to why they and their compatriots do not attend. Methods Eight focus group discussions (FGDs) were conducted with Danish and Norwegian immigrant women living in Stockholm. The women were between 26 and 66 years of age at the time of the FGDs, and were aged between <1 and 48 years old when they immigrated to Sweden. A FGD guide was used, which included questions related to cervical screening, and obstacles and motivators to attend cervical screening. The FGDs were tape recorded and transcribed, and the results analysed according to the principles of qualitative content analysis. Results The main theme was "Women have a comprehensive rationale for postponing cervical screening, yet do not view themselves as non-attenders". Investigation of women's rationale for non-attendance after being invited to cervical screening revealed some complex reasons related to immigration itself, including competing needs, organisational and structural factors and differences in mentality, but also reasons stemming from other factors. Postponing attendance at cervical screening was the category that linked all these factors as the reasons to why women did not attend to cervical screening according to the recommendations of the authorities. Conclusions The rationale used to postpone cervical screening, in combination with the fact that women do not consider themselves to be non-attenders, indicates that they have not actively taken a stance against cervical screening, and reveals an opportunity to motivate these women to attend.

    Abstract [en]

    Most work on activity-based working centers on the physical environment and digital technologies enabling flexible working. While important, we believe the key components for implementing activity-based working are employee and manager behaviors. To measure the degree of enactment of activity-based work, based on workshops with experienced practitioners as well as previous literature, we have developed and validated a behavior-focused measure of activity-based working behaviors. In our initial sample (Sample 1, N = 234), three subscales were identified: task - environment crafting, workday planning, and social needs prioritization. In the replication sample (Sample 2, N = 434), this model also showed adequate fit. Moreover, task - environment crafting was related to general health and lower stress in sample 1 (multi-organization sample), but not in the single-organization sample (sample 2). Workday planning was associated with higher concentration in both samples and in the second sample with general health and work engagement; the latter was also related to social needs prioritization.

    Denna studies syfte är att nå en djupare förståelse för upplevelsen och hanteringen av psykisk ohälsa bland män, specifikt andra generationens invandrare från Mellanöstern med omnejd men hänsyn till deras kultur. I och med att studiens syfte och frågeställning baseras på individers upplevelser har en hermeneutisk metod ansetts vara mest lämplig. Studiens syfte har uppnåtts med hjälp av semistrukturerade intervjuer präglade av teman som kultur, maskulinitet och psykisk ohälsa. Teorin om dramaturgi, medicinsk sociologi samt teorin om skam och sociala band låg till grund för studiens teoretiska ramverk. Med det teoretiska ramverket och intervjuerna som grund framställdes resultatet där en nyanserad analys med teman som skam, maskulinitet, kultur som katalysator och fasad tolkades. Resultatet visade att männen upplever en förstärkt maskulinitetsnorm till följd av att tillhöra en kultur från Mellanöstern med omnejd. Den förstärkta maskulinitetsnormen skulle visa sig påverka alla deltagares upplevelser och hanteringen av den egna psykisk ohälsan. Resultatet påvisade även vikten av fortsatt forskning om urvalsgruppen i relation till psykisk ohälsa.

    Abstract [en]

    Purpose: Psychopathic traits are associated with multiple negative outcomes. The present prospective, longitudinal study identified associations of childhood factors with adult psychopathy scores.

     Methods: 311 men, aged, on average, 33 years, were assessed using the Psychopathy Checklist-Revised (PCL-R). Predictors included neighbourhood deprivation, parents' characteristics, teacher ratings of behaviour at ages 6, 10 and 12, and academic performance at age 12. Hierarchical linear regression models were computed to identify predictors at different ages of PCL-R total and facet scores. 

    Results: Age 33 PCL-R total and facet scores were significantly, and independently, associated with father's and mother's criminality and mother's age at participant's birth when teacher ratings of childhood behaviours and mathematics marks were included in the models. Anxiety was negatively associated with facet 1 scores at age 6. At age 12, 22% of the variance in facet 2 scores was predicted by father's violent convictions, mother's age and criminal charges, and reactive aggression. Facet 3 scores were associated with mother's age (marginally), inattention, and reactive aggression. Facet 4 scores were associated with father's violent criminality, mother's age, conduct probleins, inattention, and reactive aggression. 

    Conclusion: Etiological research and prevention programs should focus on antecedents of psychopathic traits present in early childhood.

    Aim: The aim of this study was to describe the care of patients assessed as not in need of emergency ambulance care, from Registered Nurse's lived experiences. Background: Non-emergency patients in need of ambulance care are described as vulnerable and patients in ambulance care have earlier been found to be dependent on the Registered Nurse. However, little is known about the care of non-emergency patients in the ambulance setting, from the perspective of Registered Nurses. Methods: A reflective lifeworld research design was chosen. Five Registered Nurses with experience of ambulance care were individually interviewed. Results: The result reveals the essence of the phenomenon as a desire to provide good care in an on-going struggle between one's own and others' expectations. Three meaning constituents emerged in the description; Being in a struggle between different expectations, Being in a questioned professional role, and Being in lack of support and formal directives. Conclusion: Registered Nurses' care for patients assessed as not in need of emergency ambulance care, is a complex struggle between different expectations. This may be related to the encounter between the nurse's and the patient's lifeworld.

    Assistive technology and welfare technology explorations: Aspects of perception, utilisation and decision-making2021Doctoral thesis, comprehensive summary (Other academic)
    Abstract [en]

    Aims: The overall aim of this thesis was to explore decision makers’ and professionals’ perceptions, utilisation, and decision-making about assistive technology (AT) and welfare technology (WT) within the interprofessional field of care for older adults. Methods: A literature review was conducted, including nine peer-reviewed scientific articles (I), analysed with a qualitative descriptive method. A web-based questionnaire, answered by 393 officials and health care providers in municipal eldercare (II and III), was analysed using content analysis of the open-ended answers (II) and descriptive and inferential statistics of the closed-ended answers (III). A semi-structured interview with 24 decision makers was also conducted and analysed with thematic reflexive analysis. Results: The studies revealed a lack of evidence-based decision models to support assistive and welfare technology decisions, partly due to the context being complex and multifaceted. The technology was perceived as progressive and cutting-edge, while organizational structure and culture were perceived as regressive and resistant to change. The results revealed personnel working with WT were generally very positive toward new technology, although there were age, gender, participation, and professional differences. Study IV results revealed that supportive aspects, technology aspects, patient aspects, and knowledge aspects influence AT-organizations' decision-making. Although AT/WT has been portrayed to have the potential to solve the challenges of an aging population, the decision-making regarding AT/WT is complex and evolving. Increased communication between government, regions, and municipalities to support the development of guidelines would enhance the goal of a person-centred care. Conclusion: This thesis contributes to knowledge of decision-making models, focusing on GATE´s model (Khasnabis et al., 2015) applicable in AT/WT. Further development and deepen knowledge in the area of AT/WT appears as needed due to results showing that there is a lack of structured knowledge about the decision-making processes and evaluations of effects of AT/WT.

    Abstract [en]

    Background: Swedish municipalities are facing demographic challenges due to the growing number of older people and the resulting increased need for health care services. Welfare technologies are being launched as possible solutions for meeting some of these challenges.

    Objective: The aim of this study was to explore the perception, experimentation, evaluation, and procurement of welfare technology practices among professionals working in municipal elder care in relation to their gender, age, and profession.

    Methods: Data for this explorative cross-sectional study were collected from 393 responses to a web-based survey on municipal elder care in Sweden. Chi square tests were performed to determine the associations.

    Results: The results revealed gender, age, and professional differences in perspectives of municipal elder care workers. Differences were particularly evident in attitudes toward technology, both the use of technology in general and in the workplace, and involvement and participation in decision making regarding the procurement of new welfare technologies. Men (37/53, 70%) expressed a more positive attitude toward and curiosity regarding new technologies than women (157/336, 46.7%) (P=.03). Regarding age, the younger respondents (18-24 years old) perceived the digital transformation in the workplace as “too slow” (4/4, 100%), whereas the majority of older respondents (65-74 years old) perceived it as happening at the “right pace” (4/7, 57%). The elder care personnel felt encouraged by management to explore and experiment with new welfare technologies, but never did so either for management or with patients. Even though the majority of the respondents were women, more men (4/7, 57%) were involved in the procurement process for welfare technology devices and solutions than women (98/336, 29.2%) (P<.001).

    Conclusions: Personnel working within municipal elder care were generally very positive toward new technologies. However, both gender and age differences may influence these perspectives such as the personnel’s resistance to welfare technology and patients’ participation in welfare technology usage and deployment. Different levels of participation in the decision-making process regarding new technology deployment may negatively affect the overall digital transformation within municipal elder care.

    Abstract [en]

    Background: Due to the increasing ageing population worldwide, humanity is facing global demographic challenges. For many people, their later years are often lived with changed functioning and the need for support in daily living activities. Assistive technology and welfare technology (AT/WT) constitute a partial solution to the demographic challenges. Objectives: The purpose of this literature review is to identify the policy features, such as decision models and guidance for the provision of AT/WT as reported in the current scientific literature. Methods: A literature review with a deductive approach using the structure of the Global Cooperation on Assistive Technology (GATE) model was conducted. The data were searched from the databases: PubMed, Cinahl Plus, and the Web of Science, resulting in nine included articles. Results: The findings are presented according to the structure of the eight domains from the GATE model— policy, assessment, procurement, technology, environment, usability, sustainability and rights— which are further grouped into categories exploring the variations in every domain. Conclusion: The review reveals a nascent and growing interest in the area; however, evidence-based decision models and other guidance on AT/WT are lacking. Recent scholarships and policy reforms can form the basis of stronger guidance to organise complex AT/WT systems, but further and future research is needed.

    Assistive and welfare technology (AT/WT) has been introduced as a way of facing an ageing population and providing support for older adults in their daily lives. There is much research concerning the assessment and recommendation of AT/WT to individual end-users. However, few studies have explored AT/WT decision-making from a managerial perspective. This study explores what aspects influence decision-making in assistive technology organizations concerning new technology procurements. The study is based on interviews with 24 managers engaged in assistive technology organizations, representing 13 of 21 regions in Sweden. The interview data consisted of the participants’ experiences deciding on AT/WT procurement. A reflexive inductive thematic analysis was used to identify aspects that influenced decision-making. The main findings show that decision-making is in the making, meaning that decision-making is a constant on-going managerial process. Furthermore, the findings show that managers experience uncertainty in the decision-making, sometimes make ad hoc decisions and request an evidence-based, person-centred approach to improve decision-making. The study concludes that supportive, technology, patient, and knowledge aspects influence managers’ decisions.

    Idag är 45 procent av den arbetande befolkningen kvinnor. Dock harandel kvinnor i chefspositioner inte ökat. Detta eftersom kvinnor ihögre utsträckning avbryter karriären av familjeskäl. Studienundersökte om kvinnor upplevde att barnafödande och jämställdfördelning av ansvaret i hushållsarbete och barnomsorg haftinverkan på karriärutveckling. Undersökning har utgångspunkt i tvåområden; hemmet och arbetsliv. Kvalitativa intervjuer med åttakvinnor var grunden för datainsamling. Respondenterna hade enledande- eller chefsposition. Resultatet av en tematiskinnehållsanalys visade att ett jämställt hem med lika ansvar kan bidratill kvinnors karriärsutveckling. Respondenterna ambitioner omhögre lön och bättre arbetsposition ersattes med önskan om flexiblaarbetstider och möjligheten att jobba hemma vid behov efter de fickbarn. Statistiken som visar att ojämn fördelning avföräldraledigheten skulle ha en inverkan i kvinnors karriärutvecklingkan inte bekräftas i denna studie.     Abstract [en]

    Health impact assessments (HIAs) were first introduced in Sweden in the mid-to-late 1990s, with the aim of placing health issues on the political agenda and helping to reduce health inequalities. In the early 2000s, HIAs entered a second phase and the Swedish Parliament adopted a national public health policy. A national survey conducted in 2001 showed that 10/289 municipalities had begun to use HIA and 55/289 had decided to use HIA or had initiated an adoption process. In a 2013 follow-up study based on a strategic sample of municipalities, 9/36 municipalities reported using HIA and/or similar tools. Corresponding figures for the 21 Swedish regions were 10 regions in 2001 and four in 2013. HIA and similar tools (sustainability analyses, child impact assessments, and others) were applied to the same extent as HIA. Fifteen years after implementation began, HIA is still being used. Regions show a clear decrease in the use of HIA. There are several explanations for this development. One is the political context, and other explanations are shifts in which actors are responsible for HIA and for public health at the local/regional levels.

    A public health work is focused on health promotion or prevention efforts, which can be done at the local level towards the individual. Addiction units is responsible for providing support and treatment to people who have an addiction or dependence on unhealthy lifestyles. They are also responsible for improving and developing their methods and tools. However, With the development of welfare technology, Social workers in addiction units, have increasingly begun to use e-health tools in treatment work.

    It is related to vision-health 2015, which among other things, focuses on work in the health and care sector at the individual level. The digital work environment aims to facilitate everyday work that supports the processes in which social workers participate, which can ultimately contribute to the individual being offered digital support tools to improve their health and increase their participation.

    The aim of this study was to investigate social workers’ experiences using welfare technology in the form of “e-health tools” in their work. The method was qualitative with an inductive approach where the purposeful sampling was supplemented with snowball sampling. The interviews were conducted in semi-structured form and analyzed based on a manifest qualitative content analysis. The theoretical perspective of the study consists of Antonovsky’s theory SOC, sense of coherence.

    The analysis resulted in two categories with four subcategories and shows social worker’s experiences that e-health tools in treatment work contribute to increased opportunities that have facilitated motivational work and such as increased collaboration with the unit for children and families, treatment homes and occupational health care. The results also show social worker’s experiences that e-health tools in treatment work create additional challenges in the form of additional tasks and an increased situation-adapted use according to, for example, the client's mental illness.

    Studies investigating differences in mental health problems between self-employed and employed workers have provided contradictory results. Many of the studies utilized scales validated for employed workers, without collecting validity evidence for making comparisons with self-employed. The aim of this study was (1) to collect validity evidence for three different scales assessing depressive symptoms, emotional exhaustion, and sleep disturbances for employed workers, and combinators; and (2) to test if these groups differed. We first conducted approximate measurement invariance analysis and found that all scales were invariant at the scalar level. Self-employed workers had least mental health problems and employed workers had most, but differences were small. Though we found the scales invariant, we do not find them optimal for comparison of means. To be more precise in describing differences between groups, we recommend using clinical cut-offs or scales developed with the specific purpose of assessing mental health problems at work.

    Flourish, fight or flight: Health and well-being in self-employment over time-associations with business success2019Conference paper (Refereed)
    Abstract [en]

    Purpose: Around 9% of the working population in Sweden consists of self-employed business owners, but a considerable amount of them struggle to consolidate or expand their businesses. Among the factors predicting business success the decisive role of long-term health of business owners has been acknowledged only recently, but longitudinal studies testing this assumption are scarce. Based on the conservation of resources theory, good health can be seen as a resource that helps business owners to tackle high workloads and make business succeed.Design: Data from the Swedish Longitudinal Occupational Health Survey is used. Starting in 2012, N= 554 self-employed have answered three or more times in the biannual data collection. Latent growth curve modelling is employed to study general and mental health trajectories and their associations with business survival over time.Results: Preliminary descriptive analyses on biannual changes suggest that roughly one in ten self-employed leaves self-employment at follow-up. Job demands and emotional exhaustion are higher among those who leave compared to those who remain in business. After integrating new data collected in 2018, growth curve analyses are run over the whole longitudinal sample, and associations of health trajectories to business survival will be tested.Limitations: Data is collected with questionnaires, and business success is operationalized as business survival only.

    The aim of the present study was to identify and describe intensive care nurses' different conceptions of a critical pathway in caring for patients that have undergone aortic-surgery. Individual semi-structured interviews with eight specialist registered nurses at a Swedish intensive care unit were conducted and phenomenographically analysed. Three descriptive categories, with a total of five sub-categories, constituted the outcome-space of how the pathway was conceived of in caring: as a guide open to individual patients needs (clinical judgement governs caring and patient autonomy governs caring), as an instrument to promote patient safety (a source of knowledge, a planning tool and a reference standard) and as a source of support for professional confidence. In accordance with current literature, the nurses in the present study identified a number of advantages in applying the pathway in caring even if they were also conscious that the use of a pathway can give rise to unreflective standardisation. The nurses' conceptions indicate that the pathway prescribed for managing patients who have undergone aortic surgery is supportive and facilitates patient safety without jeopardising respect for the patient's individual care needs. This insight may be used to influence a thoughtful dialogue about the practice of pathways in intensive care.

    Rationale, aims and objectives Clinical pathways (CPs) can improve quality of care on intensive care units (ICUs), but are infrequently utilized and of varying quality. Knowledge regarding factors that facilitate versus hinder successful implementation of CPs is insufficient and a better understanding of the activities and individuals involved is needed. The aim of this study was to explore the implementation process of CPs within the context of ICUs. Methods An exploratory design with a sequential mixed method was used. A CP survey, including all Swedish ICUs, was used to collect quantitative data from ICUs using CPs (n = 15) and interviews with key informants (n = 10) were used to collect qualitative data from the same ICUs. Descriptive statistics and qualitative content analysis were used, and the quantitative and qualitative findings were integrated. Results The CP implementation was conceptualized according to two interplaying themes: a process to realize the usefulness of CPs and create new habits; and a necessity of enthusiasm, support and time. Multiple factors affected the process and those factors were organized in six main categories and 14 subcategories. Conclusions Bottom-up initiatives, interprofessional project groups and small ICUs seem to enhance successful implementation of CPs while inadequate electronic health record systems, insufficient support and time constrains can be barriers. Support regarding the whole implementation process from centralized units at the local hospitals, as well as cooperation between ICUs and national guidance, has the potential to raise the quality of CPs and benefit the progress of CP implementation.

    Objective: Foot impairments are related to reduced mobility and participation restrictions in daily activities in patients with established rheumatoid arthritis (RA). The new biologic medications are effective and reduce disease activity, but not disability to the same extent. Foot impairments are assumed to be related to participation restrictions also in patients with early RA, diagnosed after the introduction of biologic medications. Knowledge of foot impairments needs to be explored further after the introduction of biologic disease-modifying antirheumatic drugs (bDMARDs). The aim of this study was to explore the patients’ perspective of foot impairments related to early RA. Methods: The sample included 59 patients (ages 20–63 years) who were interviewed about participation dilemmas in daily life using the critical incident technique. The interviews were audio-recorded and transcribed. Data related to foot impairments were extracted and analyzed thematically. A research partner validated the analysis. Results: Patients with early RA described a variety of participation restrictions related to foot impairments: foot hindrances in domestic life, foot impairments influencing work, leisure activities restricted by one's feet, struggling to be mobile, and foot impairments as an early sign of rheumatic disease. Conclusion: There is a need to focus on foot impairments related to early RA, and for health care professionals to understand these signs. A suggestion for future research is to conduct a longitudinal followup of foot impairment related to medication, disease activity, and disability in patients diagnosed after the introduction of bDMARDs.

    Background: Medical errors are reported as a malpractice claim, and it is of uttermost importance to learn from the errors to enhance patient safety. The Swedish national telephone helpline SHD is staffed by registered nurses; its aim is to provide qualified healthcare advice for all residents of Sweden; it handles normally about 5 million calls annually. The ongoing Covid-19 pandemic have increased call volume with approximate 30%. The aim of the present study was twofold: to describe all malpractice claims and healthcare providers’ reported measures regarding calls to Swedish Healthcare Direct (SHD) during the period January 2011–December 2018 and to compare these findings with results from a previous study covering the period January 2003–December 2010. Methods: The study used a descriptive, retrospective and comparative design. A total sample of all reported malpractice claims regarding calls to SHD (n = 35) made during the period 2011–2018 was retrieved. Data were analysed and compared with all reported medical errors during the period 2003–2010 (n = 33). Results: Telephone nurses’ failure to follow the computerized decision support system (CDSS) (n = 18) was identified as the main reason for error during the period 2011–2018, while failure to listen to the caller (n = 12) was the main reason during the period 2003–2010. Staff education (n = 21) and listening to one’s own calls (n = 16) were the most common measures taken within the organization during the period 2011–2018, compared to discussion in work groups (n = 13) during the period 2003–2010. Conclusion: The proportion of malpractice claims in relation to all patient contacts to SHD is still very low; it seems that only the most severe patient injuries are reported. The fact that telephone nurses’ failure to follow the CDSS is the most common reason for error is notable, as SHD and healthcare organizations stress the importance of using the CDSS to enhance patient safety. The healthcare organizations seem to have adopted a more systematic approach to handling malpractice claims regarding calls, e.g., allowing telephone nurses to listen to their own calls instead of having discussions in work groups in response to events. This enables nurses to understand the latent factors contributing to error and provides a learning opportunity.

    Studien vill belysa hur genusstrukturer är inbäddade i reklam och marknadsföring i det offentliga rummet i den centrala staden. Syftet är att synliggöra hur genus skapas i reklam och skyltning i Sverige 2016, vilka semiotiska resurser som används och därmed synliggöra de sociala förväntningar som finns kring ett visst kön. För att göra detta används en diskursanalys av framförallt bild, men även text, för att identifiera vilka konstruktioner som görs. Huvudfokus är att belysa vilka framställningar som förstärker ett visst genus och att strukturera upp på vilka sätt de liknar eller skiljer sig från varandra. Studien utgår ifrån ett socialkonstruktionistiskt perspektiv och har ett fokus på språkets konstruerande och socialiserande verkan. Genom att relatera studien till tidigare forskningsresultat kan genuskonstruktionens förändring jämföras och synliggöras. Resultatet visar att kvinnor och män framställs på alltmer lika sätt och att könsnormativa överskridanden förekommer, men att genus på vissa plan fortfarande framställs på olika sätt beroende på kön. Studien visar att det både sker reproduktioner och förändringar i förhållande till traditionella framställningar av kön, vilket bland annat visas genom kroppen, dess positionering och rörelser, men även förmedlas genom uttryck av allvarlighet, upptagenhet och intimitet.

    The working conditions of nurses in the Swedish healthcare system are often debated in the news media, where the various stakeholders' (nurses, union, politicians and healthcare managers) versions of events are laid out. By using social media, nurses can participate in the debate and in a political process by drawing on narrative elements as responses to politicians' and healthcare managers' neglect of the nurses' situations. This article reports on a study of 50 topic driven blogs written by nurses from 2013 to 2015 and published on the website of a union magazine for healthcare professionals. Departing from a discursive narrative approach, the aim of the study is to analyse how the nurses narrate their working conditions and what they convey. The study shows that the bloggers create a we-ness by constructing and displaying their professional identity, illustrating unfair working conditions, conveying criticism and negotiating responsibility and blame. Addressing other nurses, the bloggers presume that others in the profession experience similar things in their workplaces and thereby encourage them to take action and make the injustices known. In order to gain credibility for the reported events and the relations, rhetorical resources such as factual accounts, categories, pronouns and a media discourse are used. The media discourse that is (re)produced in the blogs and elsewhere may result in nurses being trapped in their way of talking about their working situation, normalising it and having difficulty finding solutions.

    This article focuses on how the Swedish news media depicts the functioning of the social services and the situation of social workers. The empirical material consists of 586 articles published online during the years 2010–2015. A discursive approach makes up the theoretical and methodological framework. The aims of the article are to identify and categorise media narratives about social services and social workers, how they are portrayed, what is conveyed and how the stakeholders’ (politicians, managers and social workers) negotiate the question of responsibility for the situation in the social services. The study identifies and categorises seven different types of media narrative. In these narratives, social workers are portrayed in various ways, such as unwilling to stay in the profession, concerned, hardworking, etc. Reports on the crisis in the social services tend to dominate, e.g. how the social services fail to follow-up cases of child abuse, social workers’ heavy workload and that legal security cannot be guaranteed. Stakeholders, faced with reported criticism, maintain credibility by taking some responsibility and/or displacing liability by using various rhetorical resources. The media produces a public image of the lack of responsibility for solving the problematic situation. In this respect, the take-action-narratives are central to the debate, which argues for joint action to be taken to find solutions. Although the media is a powerful and scrutinising tool, it also needs to report on the improvements that have been made in order to give a more balanced picture of the social services.

    The aim of the article is to problematise and discuss the usefulness of the chronological I as a new analytical approach for studying the doing of identity in storytelling. The chronological I can be both a rhetorical resource for narrators and a new analytical tool for studying the process of doing identity. The article suggests that the chronological I adds a new analytical dimension to different types of narrative analysis. The article takes its point of departure in the understanding of the narrator as using time as a rhetorical resource for telling or doing identity in ongoing interactions. In this discursive narrative approach, narratives are viewed as socially situated actions in a context in which the narrator has to relate to culturally accepted agreements about responsibility and agency. The data for this article is based on interviews with twelve individuals exposed to workplace bullying. As this topic is sensitive, there is a need for narrators to manage their accountability when asked to account for their agency or non agency in the reported events.

    The aim of the article is to shed light on the social dynamic that characterizes communicative breakdowns. As a first stage, this has been explored through individual and group interviews with 34 healthcare professionals about situations where they claim there has been a communication breakdown. Based on the professionals’ testimonies, the research question was: how do professionals illustrate their experiences of communicative breakdowns? In terms of theory, the study draws upon an integrated approach to language and social interaction dynamics – for example how actors mediate positions and roles, but also present, negotiate and determine the course of action in patient cases. Findings show: (1) different degrees of severity in communicative breakdowns, and (2) that the professionals have developed and use both explicit and implicit strategies to handle ”delicate relationships” in this kind of interactions.

    The right-wing Swedish Nordic Resistance Movement (NRM) is increasingly active on social media. Using discursive psychology, this text explores the rhetorical organization of text and rhetorical resources used on the Swedish online forum Flashback. The aim is to reveal and problematize truth claims about NRM made by antagonists and protagonists. Questions are (1) how and what do NRM antagonists and protagonists convey in Flashback posts about NRM, and its ideology and members? (2) how do NRM antagonists and protagonists make truth claims about NRM in Flashback posts? The empirical material consisting of 1546 Flashback posts analyzed to identify typical discussions on "NMR's true nature"; accomplished social actions stemming from the posts. Findings show that the Flashback thread can be understood as being a rhetorical battle that concerns the "truth" about NRM, where a variety of rhetorical resources are used to render statements credibility and those involved legitimacy.

    The launching and establishment of a social problem: An analysis of the debate on Swedish national level gymnastics in Dagens Nyheter 2012-2013

    In 2012 Sweden’s largest daily newspaper, Dagens Nyheter, published a number of articles on the state of affairs in Swedish national level gymnastics. In these articles, ex-gymnasts, coaches, parents and physicians stepped forward and testified about recurrent wrongdoings and abuse against young (particularly) female gymnasts. In response to the criticism, the accused coaches and representatives of the Swedish Gymnastics Federation downsized or dismissed the criticism as inaccurate. This being said, and using discursive psychology and a qualitative design, this article sheds light upon this debate – by viewing it as a struggle between a hegemonic discourse of ”the goodness” of sports, on the one hand, a number of unfavorable and negative testimonies of the state of affairs in Swedish national level gymnastics on the other. More concretely, this struggle has been analyzed with regard to the discursive and rhetoric resources the involved parties’ used to pursue their claims about the state of affairs in Swedish national level gymnastics and the impact these resources had for their credibility and legitimacy. Questions were: Who is entitled ”to spell out” their view on the state of affairs within Swedish national level gymnastics? What is mediated and how? How are descriptions and accounts about reality constructed as credible and factual? It is concluded that social problems are launched via co-production; in this process, the gymnastics community, the research community, single individuals, and the media – were co-actors.

    Aim: The aim of the study is to uncover and reveal how nurses as a profession and their working conditions are dramatized and portrayed inSwedish media narratives about the shortage of nurses. Background: The media is an arena where stakeholders can air their views of the healthcare sector in general and the situation for nurses in particular. The focus in this study is the debate in Sweden on the shortage of nurses. Design: Qualitative discursive study. Method: A discourse analysis of media narratives about nurses and their working conditions published in several Swedish newspapers from 2009-2014. 1779 articles were included in the study. A selection (113 articles) of these articles was further analysed using a qualitative discursive psychological approach. Findings: Nurses are portrayed as being good, concerned about and critical of healthcare managers and politicians for not taking action. The accused actors justify their actions by partially accepting or displacing responsibility. The shortage of nurses is framed as a social problem - a threat to patients' safety. Seven different types of metaphorical expression frame the problem as inevitable, beyond control, abstract, an individual and collegial problem and nurses as replaceable. In addition, nurses and patients are dehumanized and no-one is held responsible. Conclusions: This study analyses the role of the media in emphasizing the seriousness or obscurity of the problem and possible solutions to it. Alternative narratives are needed to re-frame the nursing shortage and to find sustainable solutions.
