    Abstract [en]

    In Sweden the risk for female Anorexia Nervosa and Bulimia Nervosa is estimated to 1%. Females have ten times higher prevalence than males. Attention Deficit Hyperactivity disorder (ADHD) on the contrary has a male 2-3 times dominante. Among adults aprox. 3% has ADHD. ADHD is complex with at least one major associated psychiatric diagnosis. The aim of the present study was to investigate whether clinical findings, not just related to Eating Disorder (ED), could match ADHD as a co-existing diagnosis, as well as to follow the patients the first months after treatment with Methylfenidate had been induced. Five patients, one male and four females, age 18-43, with a history of Eating Disorder (ED) of 5-35 years, were clinically described with DSM-IV diagnoses, treatment regimen and therapeutic outcome. They suffered from disability in normal social functioning, i.e. education, professional career and family life. All of them had symptoms matching ADHD. In this case report the assumtion is made that treatment with Methylfenidate lead to an decrease of ADHD-symptoms as well as typical ED-symptoms, better social functioning and increased quality of life.

    The application of economic incentives to providers in health care governance is debated. Advocates argue that it drives efficiency and improvement, opponents claim that it leads to unintended consequences for patients and professionals. Research shows that incentives can increase well-defined activities and targets, but there is a lack of substantial evidence that applications in health care lead to desired outcomes. The motivational literature acknowledges internal sources of motivation as important determinants of behavior, and the literature about professions suggests that professional values of serving patient needs is a key motivator. The management literature identifies the important role of leaders in aligning external demands and rewards to staff preferences, using their own management and leadership skills. Findings in health services research confirm the vital role of leaders for successful implementation and improvement work. In sum, internal motivators and the role of leaders are important to acknowledge also when understanding how economic governance models are put into practice. Our recently published qualitative case study provides empirical examples of how clinical leaders function as intermediaries between a local care choice model, including financial incentives, and the motivation of staff. The strategies deployed by the leaders aimed to align the economic logics of the model to the professional focus on increasing patient value. The main conclusion from these empirical examples, as well as previous research, is that health care managers play a key role in aligning economic incentive models with professional values and in translating such models in to feasible tasks related to the provision of high quality care.

    [Pioneers' work against odds resulted in two new medical specialties]

    Torsten Gordh Sr and Ragnar Berfenstam are two influential Swedish doctors, who have worked in medical specialities of quite different character, namely social medicine and anaesthesiology. Still they have much in common: both introduced a new medical speciality in Sweden and both continued with research for many years after retirement. Two physicians with this background should have much to offer young doctors of today. We therefore performed an interview study to describe Torsten Gordh's and Ragnar Berfenstam's experiences as researchers and physicians. The study was based on in-depth interviews. The thematic analysis of the interview texts showed that the two pioneer physicians had to fight an uphill battle to introduce their new medical specialities. Both started new fields of scientific research; at the same time both were dedicated to their professional work. Torsten Gordhs and Ragnar Berfenstams were not only successful researchers but also proficient and considerate doctors for their patients.

    Recently interactions between promoter polymorphisms in the serotonin transporter gene and the Monoamine oxidase-A gene have been found to interact with psychosocial factors to predict outcome such as adolescent criminal behaviour, alcohol consumption and depression. In this paper we review this emerging field of scientific inquiry with particular attention paid to findings made on a population based sample of 119 girls and 81 boys from the county of Västmanland, Sweden.

    Despite the fact that emergency contraceptive pills (ECP) have become easily available across the country during recent years, abortion numbers continue to rise in Sweden, especially in the young age groups (< 25). In a series of studies, we have investigated knowledge, attitudes and experience of ECP among young women. Our results show that, whereas most women are aware of the method, many lack knowledge about the mechanism of action and time frames for best use, which could explain why ECPs are not used by more than a fraction of women who might have had benefit from their use. Since half of the women requesting a termination of pregnancy stated that they would have used ECP if they had had them available at home at the time of the unprotected intercourse which led to an unintended pregnancy, it seems reasonable to encourage women to keep ECPs at home, in case the need should arise. It is important that ECPs are available without prescription, but beyond that, much more information about ECP is necessary in order for the method to be widely accepted and used as a back-up after failure with other contraceptives.

    Implementation of innovations – social, technical and scientific – often fails. Implementation science is the multi-disciplinary field that addresses how implementation can succeed. This article summarizes some basic premises in four questions: 1) Is the innovation the best solution to an important problem? 2) What behavior changes does the innovation require? 3) What strategies are needed to make the changes possible, probable and lasting? and 4) How is the innovation used? In this, implementation is positioned as a way to create a fit between the innovation and the context. It highlights that the end result depends on the innovation, the context and the implementation strategies together.
