SAMMANFATTNING

Barn- och ungdomspsykiatrin påverkas av flera aspekter i omvårdnaden då den aktuella vårdkulturen är komplex. Studiens syfte var att belysa innebörden av vårdkultur inom sluten barn- och ungdomspsykiatri utifrån sjuksköterskans perspektiv. Studien genomfördes med fokusgruppsintervjuer vilka utgörs av öppna gruppintervjuer med fokus på avgränsat ämne. Intervjuerna transkriberades och analyserades enligt en fenomenologisk-hermeneutisk metod. Teman som visade sig var: Vårdkulturens övergripande innebörd, vårdkulturens kontextuella innebörd, vårdkulturens förmedlande innebörd samt vårdkulturens innebörd vid relationer. Resultatet visar att vårdkulturens innebörd utgörs av enighet och splittring både beträffande vårdkulturen som helhet, men även delar av den. Strävan efter enighet är genomgående, samtidigt som splittringen är oundviklig för av skapa formbarhet. Vidare besitter vårdkulturen en guidande funktion för sjuksköterskan både medvetet samt på ett omedvetet plan. Detta får konsekvenser för sjuksköterskan, omvårdnaden och patienten. Endast genom att vårdkultur synliggörs för sjuksköterskan kan en medveten hållning ske genom reflektion och dialog.

Abstract [en]

ABSTRACT

Child and adolescent psychiatric care is affected by many different aspects of care when the current nursing culture is complex. The aim of the study was to highlight the essence of nursing culture in the in-patient child and adolescent care from a nurse’s perspective. The study was conducted using focus group interviews which consist of open group interviews with a focus on the specific topic. The interviews were transcribed and analysed according to a phenomenological-hermeneutical method. Themes that emerged were: Nursing culture overall essence, nursing culture contextual essence, nursing culture’s intermediary essence and the essence of nursing culture in relationships. The result shows that the essence of nursing culture is characterized by unity and division both in respect of nursing culture as a whole, but also in different aspects of it. The quest for unity is consistent, while the division is needed to create plasticity. Furthermore, nursing culture possesses a guiding function for a nurse on both a conscious and an unconscious level, which have clear implications for both the nurse, the nursing care and ultimately the patient. Only by making the nursing culture visible to the nurse, a conscious posture can begin through reflection and dialogue.
