Bakgrund: Patientöverlämning är en komplex uppgift där mottagande vårdpersonal förväntas delges viktig information om patientens tillstånd. Det läggs vikt vid att arbeta riskförebyggande för att upprätthålla hög patientsäkerhet. Användandet av kommunikationsverktyg minskar risken för att viktig information försvinner vid överlämningen. När kommunikationen vid postoperativa överlämningen är otillräcklig ökar risken för allvarliga komplikationer. Personcentrerad vård användes som vårdvetenskapligt perspektiv. Syfte: Att belysa faktorer som kan påverka den postoperativa patientöverlämningen utifrån vårdpersonalens perspektiv. Metod: Kvalitativ metod med allmän litteraturöversikt med 23 artiklar inkluderade. Resultat: Övergripande faktorer som påverkade patientöverlämningen var; kommunikation, patientens tillstånd och teamarbete och arbetsmiljö. Slutsats: Användning av välstrukturerat kommunikationsverktyg förbättrade patientöverlämningen. Genom att anpassa överlämningen efter patientens tillstånd och att teamet visar respekt för varandra kan arbetet effektiviseras. Att ha en god arbetsmiljö ansågs som viktig för att förebygga risker i patientsäkerheten. Examensarbetet anses kunna bidra till ökad kunskap hos vårdpersonal som deltar i postoperativa överlämningar. Genom kunskapen kan överlämningarna förbättras och därmed kan bättre och säkrare postoperativ vård bedrivas vilket kan höja patientsäkerheten och förbättra den personcentrerade vården.

Background: Patient handoff is a complex task where receiving personnel expect to receive important information about the patient’s condition. Emphasis is placed on working risk prevention to accomplish the best patient safety. The use of a communication tool reduces the risk of lost information during handoff. When communication at postoperative handoff is inadequate, it increases the risk of serious complications. Person-centered nursing was used as a health care perspective. Aim: To illustrate factors which affect postoperative patient handoff from the professional’s perspective. Design: Qualitative method with general literature review with 23 articles included. Findings: Overall factors affecting patient handoff were communication, patient condition and teamwork, and work environment. Conclusion: Frequent use of well-structured communication tools improved patient handoff. By adapting to the patient’s condition and by the team showing respect for its members the handoff can become more effective. A good working environment was important for preventing risk in patient safety. The presented information could contribute to improved knowledge amongst the healthcare professionals working with postoperative handoffs. The knowledge could make the handoffs better and could lead to better and safer postoperative care which improves patient safety and person-centered care.
